• Title Page – Name of proposal (Include name of agency, and people presenting the proposal)

• Summary Statement (approx. 350 words), 1 page only, single-spaced

• Statement of Need: This entails a description of what condition your agency wants to change. It addresses a problem or issue, including a risk, target, and impact population. It is very important to be specific, and include supporting statistical data in your narrative, as well as a local and national review of the problem, and literature. [Approx. 3 - 4 pages]

• Goals and Objectives: Goals are ideals and objectives represent relevant, attainable measurable and time-limited ends that will be achieved. Four kinds of goals can be prepared in your proposal: operational, process, or service, product and impact objectives.

• Program Components: Activities and Tasks: This aspect of the proposal presents a work plan of how your agency intends to accomplish its objectives. It discusses the logic and continuity of the project, describing each objective, what will be done, by whom and by when. [Approx. 2- 3 pages]

• Evaluation: It is important to be able to compare the intended results with actual outcomes. You need to be able o describe the kind of evaluation (e.g., process and/or summative evaluation) that will be done, and who will conduct the evaluation. Good, clear and doable objectives are essential. [Approx. 1 ½ - 2 pages]

• Organizational Capability: The organization’s ability to implement the project is important to the funding body. It is imperative to describe how the organization was formed, past and current activities, the support and collaborative efforts involved with other organizations, and significant accomplishments.[Approx. 2- 3 pages]

• Program Continuation: A description regarding the feasibility of the organization to continue the project beyond the grant period is described here. Because it is difficult to project into the future you may have to describe a scaled-down version of the project. [Approx. 1 ½ - 2 pages]

• Budget (must include one page narrative): While the budget is important, it is negotiable, especially if your proposal is a good one. The degree of detail regarding the budget varies, based on the funding source requirements including allowable rates, fees, per diem expenses, and travel. [Approx. 1- 1 ½ pages]

• Appendices: These are extras that will help in the understanding of your proposal such as job descriptions, affirmative action policy, list of board of trustees, statistical charts, your organization’s 501[c][3] tax exempt status from IRS, letters of support, and anything else that will enhance your proposal.
