intellectual property for a product in the US

We have a UK patient and have community designs. We started the application for IP in the US to protect us during our launch and time trading. We need help and advice with a view to filing IP. Please see attached, our first attempt to file our IP. The pandemic hit and we lost track (a director who was unwell) and we'd like toes what we can do.
