Executive summary PPT slide

I am looking for a freelancer to create an executive summary presentation slide for my project. The purpose is to provide one slide depicting the high-level, metric to goal performance data summarized in the attached spreadsheet on the "High Level Overview" tab to my internal team. The presentation can range from casual to formal as long as the message is conveyed effectively. I have included a screenshot to serve as one potential example of the desired outcome, a 1:1 copy is not a requirement. I just need it to look simple and sleek enough for a Director / Executive Director level please.
