format word doc to ebook/print for amazon publishing

I am looking for a freelancer who can help me format a Word document into an ebook and print format for publishing on Amazon. The document is between 50-100 pages long and I will be providing the design elements such as images for internal book only, no book cover right now.. simple image at top of every page in black and white centered with flourish on each side of image. This is a book of quotes, there will be 3-4 quotes per page. placed randomly from quotes in book. when awarded project will be provided the graphic for top of pages.

Skills and experience required:
- Proficiency in formatting Word documents for ebook and print publication
- Experience with Amazon publishing requirements
- Ability to incorporate provided design elements into the document layout
