Recreation of a PDF document on my own letter head

I am looking for a freelancer to recreate an official report on my own letterhead. The purpose of the document is to present important information professionally. I would like the formatting to be customized to match my letterhead, which will be provided to the freelancer. In addition, I would like my logo to be included in the document. The ideal freelancer will have experience in document formatting and design, and be able to create a clean and polished final product that matches my branding and details all the information necessary for my see operating procedure in our labs.
