Recreate Pulmonary Function Test (PFT) Report with Custom Data and Graphs (Exact Replica)

Project Description:
I need a skilled freelancer experienced in medical document design and data visualization to recreate a 4-page Pulmonary Function Test (PFT) report. The goal is to replicate the layout, styling, tables, and graphs from my original PFT PDF report exactly, but with updated values that I will provide.

This is not just a redesign, it must match my original report in appearance, structure, colors, fonts, and formatting, including all medical graphs and tables. The final result should be print-ready, in PDF format, and look identical to the original, but using my new clinical data.

What You’ll Do:

Recreate the document layout (based on the original I will provide)
Replace all numerical data in tables with my updated values
Match fonts, spacing, row heights, table borders, units of measure, and alignment
Recreate and insert clinical graphs:
Flow-volume loop
Volume-time curve
Z-score slider
Trial graphs (with color-coded lines)
Trend graph with historical values
Ensure values and units are placed precisely (e.g., parameter left-aligned, unit right-aligned)
Include LLN/ULN reference values I provide for each parameter
Requirements:
High proficiency with Adobe Illustrator, InDesign, or other professional layout tools
Ability to match complex medical formatting exactly (based on sample)
Experience plotting medical data accurately (you will be given exact values to graph)
Final deliverable: editable file (AI, PSD, InDesign, or DOCX) + final PDF

I Will Provide:
Original PFT report (PDF) for reference
All updated numerical values for each table
Reference and LLN/ULN ranges
Clarification on layout, column names, and any custom formatting needed

Deliverables:
Editable source files (AI, INDD, DOCX, or equivalent)
PDF version of the final report, professionally formatted
Separate high-res image files (PNG) of graphs (optional)
