PDF document design - annual report

Hi,
Im looking for a PDF to be designed based on 30 pages of text.

Requirement:
- front and back cover on theme of green hills and sea with company logo and title on front and company details on the back
- dividers for chapters photo + chapter title (no text on back of divider - only photo)
- headings and subheadings
- page numbering
- putting in signatures

Saving:
1. In InDesign format
2. In PDF format for print-with bleeds
3. In PDF format for digital publication - no bleeds, list of content linked to chapters

I have last year’s example for information purposes
