PDF Form Fix and Word Export

I'm experiencing issues with my PDF document. Certain fields are not accepting input, specifically text fields. I need a professional who can troubleshoot this and fix the fields in my PDF document created with Adobe Acrobat. Additionally, I would like to have the document exported to Microsoft Word.
