Logo Revision & Format Supply

I need a designer that can create a logo for us based off a font we have already chosen, icon creation based off images we have selected for it, text layout, and some aspects of the design. Once we agree on the revisions, I will need the logo supplied in various formats.

Ideal Skills and Experience:
- Graphic Design
- Logo Design
- Adobe Illustrator or similar software

The modifications will involve:
- Creation of icon or symbol
- Changes to the text layout
