Interactive form set up in InDesign

Hi there, I am looking for someone with plenty of experience in this to make the attached form interactive in InDesign. I am also a designer and I have set this up in InDesign which I will supply. I just need the form interactivity to be set up in InDesign and not Acrobat like it currently is. I have an interactive pdf example of what I want it to look like. The attached is just a flat version of the form. There are 3 forms like this in total for now and I need them quite urgently. Can you please send a price for one form and depending on how that goes, I will send the rest over. There can be more forms to follow. I would like to see an example of a form already done.
