Design the workbook (InDesign and Acrobat)

Please review the attached files carefully. They have complete instructions in regards to the InDesign version. PDF version is the same, but without lined space for writing.
The design should be done in our corporate style, example of which is attached to this project. Look at the "consentpdf" for example. There should be two final versions: one to work with online (Acrobat), and another for printing out the actual physical book (InDesign).
Payment will be split in half, 1st half after 1st revision, 2nd half after second.
