Design for a PDF Form in Adobe

I am looking for someone to design a PDF form in Adobe. I need the design converted to a JPG. It is an existing design that needs to be updated, and I would like the output to be A4 size. My requirements include a professional and creative output - someone who can bring my vision to life. The ideal candidate should have an excellent understanding of Adobe software, have experience designing forms in PDF format, be familiar with the latest design trends and have strong graphic/visual design skills.
