Create Fillable 1-Page PDF Notice From Examples

We need a single-page PDF form that serves as a form called "20 Day Preliminary Notice". This is used in Arizona construction projects like home building.
We are an Arizona company. We have the 3 page generic "20 Day Prelim Notice" form specific to Arizona. It includes all the correct legal language, statutes, fields, etc. We would like this condensed into 1 page, like the attached example used by a California company called JCCostrc.
After we pick the freelancer, we will supply our company logo and necessary info to be branded on the 1 page form. The final form will be filled out by our employees, then mailed or "served" to clients. It is not a form that clients or customers would ever fill out.
Key requirements for the form include:
- Inclusion of all the AZ info on the 3 page form.
- Design of appropriate fields.
- Include necessary PDF Signature Fields like those used in JCC example.
Should have experience in creating PDF forms and ability to design user-friendly forms. It should be easy to navigate for our employees.
The California example is missing a few things needed for use in Arizona. For instance, the CA form has 3 boxes on the left side for Owner, Prime Contractor, Construction Lender info. Arizona has those 3 plus "Hiring Party" info.
