I need to have a business card for our Pizza shop

We do pizzas and have variety of more than 20 pizzas. To take the business to next level, about giving a small size pizza free. that's why, I have decided to do a loyalty card. So, on the front of the card I need my details and on the back of the card I need it to say on every 8th order where bill is more than 1200Rs one medium size pizza is free. I would like to have the icons to be something similar to my business. Details are as below:

Note: On the back of the card please have text which says “We are happy when you tummy is happy”
Keep some space to write the customer’s name.
