Realistic Digital Self Portrait Creation

I'm in need of a skilled digital artist who can craft a realistic self-portrait of me. The portrait should be set against a detailed backdrop (skyscrapers during the daytime), not just a plain color or a transparent background.

I attached a picture of the inspiration behind the cover art that I want to have made (I only want 7 arms, holding a soccer ball, tan army boots, crown royal bottle, shaker bottle, holy bible, vape, and stethoscope), and a picture of my 15 year old self, so that the character can look more like me.
