Simple Multi Step Payment Form using Html & Jquery
Budget: £20 – £250 GBP
I require a simple muti-step payment form building using HTML / jquery.
Altogether it has 6 steps and I have an exact website that I would wish to base it on. including the CSS design elements.
Would like it to be identical.
It is a fairly simple task and would like it to link to a payment gateway like checkout.com or possibly Stripe.

Also please dont sent me any more than one sentence in a reply. All i require is one link sample to something that you have done similar (Multi-step payment form like the image).
