Front End Design For One Module

I need a page designed as per the attached pdf file. Only front end design. The page must be designed using HTML5, pure CSS and pure Javascript. Socket.IO should be used for server communication. All data would be in JSON format. The application uses camera is available to read barcode (QR CODE and standard 1D code)

Page must be responsive and scales well in computer, tablet and mobile phone.
