Website with Form Input (template is ready) with database connection and a subsite where the database entries are shown and can be edited - On Azure Cloud

Hi there i had a website template ready with a few inputs wich i need to store in a database.
Also i need a subsite on it with an table where the data from the database is shown and can be edited.

All this should run on azure (maybe a sql instanz on azure and a app for it?) if that is possible also the user login/logout should work
