Website design and implement simple calculator for WIX CMS

We need a form and simple calculation done based on user input.
We have the calculation ready to be implemented see spreedsheet,
Other requirement is that there needs to be a email field to get to see the answer to the calculation.
The information in the fields + email should be sent/ collected for us to use.
We should also "easily" in Wix be able to update the different input costs.
