Web Form that sends to API

I'm looknig for someone that can create a web form - I'll provide what the UI needs to capture.
The URL for the form should be something like this:
https://url.com/form/id/user
When people submit the form I need the information along with the "id" and "user" from the url to be put in json format and sent to an api end point (that I will provide you)
