Update a web form to ask the user directional questions before showing them the main form

I have a form for turning in suggestions and am running into issues where users are confused as to if they should be turning in a suggestion or doing something else.
I want to update the form to ask a series of questions in order to direct the user to what action they need to be taking as well as change the format of the form itself to make some of the options less confusing to the user.
I've attached the full requirements, a flow chart showing how the questions and answers should be directing the user, the page that needs to be modified, and a backup of the test database.
The database is not to be modified as the only thing that is being changed is the format of the questions and how the text of the one field is formatted.
I had to remove the packages folder due to size.
