SquareSpace 7.1 File Upload Feature in Donation Checkout Page

I would like to add the file upload feature to the donation checkout page. I have setup a custom checkout form but cannot get the file upload form feature to show in the actual checkout page. I would like all the information to be connected, when someone fills out the form and when they pay. Please confirm and show evidence that you've done this before applying to the project.
