Shopify Design & Contact Form

I'm seeking a skilled freelancer to perform the following tasks on my Shopify store:
- The design has already been created, just requires very minimal changes
- Implement custom coding as needed to support design overhaul.
- Revamp the contact form, incorporating extra fields to gather more information from customers.
Ideal candidates will have:
