Need a simple web or desktop application

Hello. I need a simple desktop or web application, to issue my reports. I'm working with teleradiology so I need some form to input my client data (like name, who is requesting the exam, telephone etc), input my report and input the images of the exame. At the end it will need to generate a PDF report with the design of my brochure.

Please just bid if you understood. Type result of 10+3-6 at beggining, I will only read this applications.
