I am seeking an experienced and highly skilled developer to create multiple online medical questionnaires as part of a new "Minor Ailments Prescribing" initiative. The primary goal of this project is to develop secure, user-friendly forms that will collect patient data, ask relevant questions, and ultimately lead to a completed form that can be securely emailed and printed out.

These questionnaires will serve as prescribing algorithms, allowing pharmacists to assess whether a patient is a candidate for a pharmacist-initiated prescription based on their responses. The forms should be designed in a way that terminates the questionnaire if the patient provides an answer that is considered a "Red Flag" – a response that indicates the need for a physician's referral, as it cannot be appropriately addressed within a pharmacy setting.

Project Requirements:

1. Develop multiple online medical questionnaires, each tailored for specific minor ailments.
2. Ensure the questionnaires are user-friendly and easy to navigate.
3. Implement a secure method of collecting and storing patient data.
4. Create a system that progresses through the questions while allowing for questionnaire termination if a "Red Flag" is identified.
5. Develop a completed form that can be securely emailed and printed out by the pharmacist.
6. Integrate the online medical questionnaires into our existing website (optional, depending on your skill set).

The ideal candidate will have a strong background in web development, experience with creating medical-related forms or applications, and an understanding of data security and privacy regulations. Familiarity with the healthcare industry, specifically the pharmacy sector, will be highly advantageous.
attached are PDF forms (all of the data is present), all that is needed is to turn it into an online form. The final product should be a filled and signed form (digital signature or a finger signature).
