Landing page design with Google QR code and dynamic url

I am looking for someone who can make a nice landing page design for desktop users.
The users can scan the QR code shown that is being generated via Google API, on which variables are being pulled from the browser's URL and passed to the QR code API. See attached word file.
Probably HTML, CSS, Design skills and Javascript could do the trick.
