Hi,
We urgently require a skilled HTML developer to convert a 2-page PDF form into a static HTML layout. The form we aim to replicate can be previewed here:

Availability & Timeline:
We are on a tight schedule and need the project completed within the next 48 hours. Can you meet this deadline?
Project Requirements:
- All text within the PDF must be converted to HTML text elements, not images.
- The HTML will be used with domPDF to populate with real data later. It should mimic a paper form and not include any interactive elements like input fields. For all areas with a "filling grid," ensure that the CSS allows for text replacement that fits within the designated boxes. The delivered HTML should include sample data (which we can provide) to validate the layout.
- All checkboxes should be represented as individual images that can be easily toggled (tick/untick) in the future.
- CSS must be embed in the HTML file, not appart in a CSS file.
- You can use free font (TTF) or google font.
