Fillable Form Webpage with Send & Sign functionality
Budget: $30 – $250 USD
I am looking for a freelancer who can create simple, text only webpage.
Page will have one function: to complete, sign and send Letter of Intent's.
We require our form converted to fillable fields that populate the Letter.
We require the ability to Sign (with finger on a phone) and the ability to send the document via Txt & / OR Email.
Capturing the data and exporting in a spreadsheet by field would be nice too.
The form will be used as a Letter of Intent and should have less than 10 fields.
This is very simple. I have a working version created with Chat GPT. The simpler the better.
We will add more Forms / Docs as this one is rolled out successfully..

- I have a specific design in mind for the form
Skills and experience:
- Web development skills
- Experience in creating fillable forms
- Familiarity with form send and sign functionality
