File Converstion

I have a database of files in rtf, word, pdf format. I need these files to be converted to web forms by using a CGI, so there are submit and clear fields commands. Also, these files have tons of data that I do not want to display at all. Can you give me an example of how you would do this? Alternatively, we could just turn these from html to interactive html. Any file where I could collect data from customers, like a really big contact form. Tell me how you would go about this, I will give you a sample file to do.
