Extract form fields into email

I am looking for a freelancer who can help me with a project to extract form fields into an email. The form fields that need to be extracted include Name, Email, Phone Number, and Address. The email is sending but it looks like this

Last name: undefined
Work email: undefined
School name: undefined
Phone number: undefined
Type of query: undefined
How can our team help you:

Skills and experience needed:
- Proficiency in HTML and JavaScript to extract form fields
- Knowledge of email integration to send the extracted information using Sendgrid

I do not require the extracted information to be stored, so no storage is needed. The extracted information will be sent to an email service called Sendgrid.
