Debug Webflow & Hubspot Form Integration

We recently launched a webflow website. The forms are native from webflow but we have added code to divert the data for submissions to hubspot instead of webflow's CMS. The issue we are running into is that hubspot is creating a new form (instead of a new entry) every time an entry is made in webflow.

We need someone to debug our set up so that we aren't making duplicate forms in hubspot with each entry.
