Custom Order Form Script for Squarespace

I'm seeking a developer to fix a custom script I've developed with Chat GPT for handling order details on a form within my Squarespace website. The current script does the main job including formatting, registering the events etc but the form fails to include the "hidden fields" input when the email is sent.
