Creation and delivery of 3 Ready to print files & Online HTML Form

Hello ! this project has been amended. I am still looking for an experienced creative professional to create and deliver 3 ready-to-print files as well as an online HTML form.
This person needs to have experience working with design software, specifically Adobe Illustrator or Photoshop, so he/she can deliver high-quality files. these files must be in .eps and .pdf in HD quality. Images in CMJN mode - 300dpi on scale 1/4
The landing HTML page needs to display correctly on all types of screens and especially on mobile phones. It will consist of a header title with a world map as background, followed by the display of the same content used for the printing files, and ending by a simple reservation form displaying a short text and allowing the visitor to send: Name - E-mail address - Telephone number - Number of articles (dropdown menu as from 5 articles). The form must also provide a simple control test (captcha or sum to be calculated manually) in order to allow validate and submit the information to our email address (our email to be filled by us when uploading the html page to our server).
The successful freelancer will be responsible for providing the final deliverables, compliant with my design/brand guidelines. I would greatly appreciate it if you could demonstrate your experience in this type of project and provide a timeline for delivery.
