Create web forms in Orbeon forms (Must have previous Orbeon experience)

Use the 1004 Desktop PDF file as a reference to create a Orbeon Form data entry form. We have a naming convention spreadsheet for fieldname consistency. We are looking for some with experience laying out the forms in Orbeon to ensure the layout is clean and usable.
