Create custom quiz that is connected to mailchimp group(s)

I am trying to create a custom skin care quiz that has several questions. One of the questions is regarding skin type and the answer to this question will add the person to a pre defined mail chimp group. There are approximately 10 questions. The quiz can be a one pager or if possible something like this.
