Create A Registration Form Using JotForm In 2 Hours Or Less

I am looking for a freelancer to create a registration form using JotForm. The form will be a copy of a previous form that was created using CognitoForms.

The new form needs to have all of the same information as the previous form, including the drop down options.
No integration with other systems or software is required for this project.
