Convert PDF to HTML by hand

Hello, I need to convert a single page PDF form to HTML template (that can then be converted back to PDF). Unfortunately, the PDF is basically an image, so it cannot be converted using PDF->HTML tools. It needs to be designed in HTML by hand using tables, divs, etc. The following rules apply:
1) HTML must be a single file. No external stylessheets or images.
2) All images and styles must be inline
3) Data will be programmatically added to the form later so each field must be clear and ready to have programming tags to be embedded in it.
4) the HTML form will later be converted back to PDF, so the PDF must look EXACTLY like the HTML as rendered in the browser and vice versa
Attached is a sample of the PDF. The real PDF will be provided once you are selected for the project.
