Build website with health plan quote form | Vanilla hmtl and js | Bootstrap framework | Nodejs

Hi, I need a basic website which should have a homepage only with a form for a health plan. It's a health insurance website. A form is required to input information from traffic, it should generate a health plan after form submission with a pop. The website should be built on Vanilla HTML, JS, and BOOTSTRAP framework.

Check the attachment for the popup sample. Also, the form should have a progress bar according to its steps.
