Build me a simple web application for private hospital

DEADLINE is 9th October

We need a simple web application for clinical trial staff to be able to observe the outcomes of trials of different treatment techniques or prescriptions in the hospital. The aim is to find the best way to diagnose, prevent, or treat diseases. The web app must be able to record any “SIMPLE” observation gathered during studies




Requirements

We don't need any fancy web design, just a VERY SIMPLE DESIGN (Don't waste your time on web design) !
The web app will be very simple since this will be only used only for PROTOTYPES !

Must be uploaded on GITHUB (so we are able to track your work)

Clinical Trial Staff (Requirements)
- Must be able to log in
- Must be able to create a patient record
- Able to create a clinical studies to group all the patient records
- Able to create a trial organization
- Must be able to view any number of enrollment by study
- Must be able to mark any patients as off study when the study is completed

Nurses (Requirements)
- Must be able to log in
- Must be able to record any observations
- Must be able to record any treatments
- Must be able to submit their answers which will be automatically marked


Code must be written in Java and stored in MySQL database !
