Build Wellness Survey/Questionnaire on Wordpress website and display results immediately. -- 2

I am looking to build a Wellness survey/questionnaire on my Wordpress website that will display the result for each category (amount of points gained). The survey has 8 categories (Emotional wellness, environmental wellness, financial wellness etc)... Each category has 10 questions (they are in Word documents for now) and each question is worth 1 PT if answered YES and 0 points if answered NO. You get the idea.
When they finish filling in the survey/questionnaire I want the person who filled in the answered to see their result immediately in a nice format, for example:
Emotional Wellness: 8 points
Financial Wellness: 6 points
Occupational Wellness: 1 points
Want it to be displayed nicely,...

But I leave it up to you to display it nicely.
I will give access to backend once I pick the right candidate for the job.
