Build Shopify Plus Website Form
Budget: $250 – $750 AUD
We need a form built on our shopify plus website. We sell medical skin care products and instead of "Add to Cart", we wish to set up a new process whereby a consultation takes place and customer submits information to determine is they can be prescribed and sold a certain product.
1. On product page, change button to "Order Now" (for certain products only, managed by product tags within our custom theme.)
2. Order Now button links to a simple web based form (to be built, 5 text based questions).
3. Upon submission, new thank you page shown and email notification sent.
I want to work with Australia based developers only so that I can talk over the phone, ask questions and explain the brief.
