Add-on feature for bootstrap webpage

I'm looking for a freelancer that can add a login page and a followup form entry page to my existing website. The values in the form must be validated and the details regarding validation will be given following bid acceptance. Upon submitting the entered form, the page must generate a pdf that can be downloaded directly. The current page is done in bootstrap js and the add-on page must be done as a subpage, without giving any redirecting buttons from current page to the new login page. This is to be done in 2 days with no overhead for error, and upon completion a google meet will be held for your explaination regarding how it was done.
