Medical Insurance Claim Form Completion

I need help finalising a single medical insurance claim. I already have the insurer’s official paper form; I will send a clear scan along with all of the patient, provider and treatment details.

Here’s what I’m looking for:
• Neatly fill out every required field on the form, ensuring all diagnostic and procedure codes, dates of service and member information match the documents I provide.
• Flag anything that looks incomplete or inconsistent so I can clarify before submission.
• Return the finished form to me as a high-resolution PDF (ready to print and mail) and, if convenient, a Word or Excel copy of the typed data for my records.

Accuracy and confidentiality are essential, so familiarity with HIPAA guidelines and medical billing terminology is a plus. The whole task should take about two hours once you have the materials; I’ll supply everything you need immediately after we start and will stay available for quick questions.
