Illawarra Medication Review (iMR)
Budget: $250 – $750 AUD
We provide a home medication review service. Where an accredited pharmacist will visit patients at their home to conduct a comprehensive review of their medications and report any identified issues to their treating GP.
The service will benefit a patient who may:
Be on more than 5 medications
Has been discharged recently from the hospital
Having difficulty using their medications ( inhalers, insulin)
On medications with narrow therapeutic window ( dose sensitive medications)
The process of this service starts when the treating general practitioner identify a patient with one or more of the above mentioned criteria and send a referral to the pharmacist to conduct the medicating review , services paid by medicare not by patient.
