order form
Budget: $30 – $250 CAD
I am looking for a freelancer to create an order form for my physical products (Tshirts).
Preferred Layout:
- I have a specific layout in mind for the order form. See attachment.
Changes:
- I need the images to include all 4 angles of a short and long sleeve shirt.
- I need my branding and fonts used.
- Change "Details" to "optional Details"
- Remove "Address" line.
- Replace "drop off" with "Okanagan delivery"
- Remove "new customer" section
- Change youth sizes to "Toddler, S, M, L, XL"
- Remove pricing section
- Change "Promise date" to "Drop dead date"
- Remove "printabe template" blurb in bottom left.
- Add "Please email vector artwork to _____" in Gotham Bold ALL CAPS at the bottom of the page.
- Remove "Business name"
- Please keep the address section, I will edit it when you are done.
- Please use Gotham Medium and Gotham Bold for everything.
- Black and orange (the orange in the astronaut file) are our most commonly used background colour sbut please use them sparingly as some customers may want to print the order form.
- I have attached my logo, favicon, and our astronaut that we use in all of our branding. You do not need to use all three, but the logo needs to be used for sure.
Special Features:
- I want the order form to include special features such as automatic calculations or custom fillable fields.
- It needs to be saved as a fillable PDF that is under 10MB.
- I will need the .ai file

- Experience in designing user-friendly order forms for physical products
