I need to get my twitch icon and banner and everything done -- 2

I'm in need of a graphic designer to bring my Twitch channel to life! I am looking for somebody who can create a gaming-themed icon and banner that incorporates my specific requirements. Bonus points if you are able to create additional twitch graphics like overlays, panels, alerts, etc. – I would love for the look and feel of my stream to be cohesive.
