design a parking pass

summarize, I would like to have a designer create a guest parking pass the building name (:........... )The pass will be provided by(...............), and their logo should be included on the pass. The pass should this information, the car license plate number, state of registration, date of expiration, and unit number. I would like the pass to be in a size of half an A4 paper, so that two passes can be printed on one page. Finally, black and white is okay .
