create a PDF file for the cover of an 11 x 8.5 photobook (including front, spine, back) from my template

I have a horizontal 11 x 8.5" photobook that I want printed by Lulu, but I have no file to upload for the cover. Lulu has very specific specs for the cover, and I need a file to give to them. (Lulu already has the book interior.) I have the photos, text/fonts, and barcode that I need for the cover, just need someone to put them all together for me into the Lulu format. I can give you the spine info and barcode when needed later; just front and back cover designs are attached for now. I can also send you the entire 24-page interior if it will help you.
