E-book cover, 5 e-mail ads, and landing page design

I have a completed e-book, I need a cover design. I have a Shutterstock image, would be glad to see similar from freelancer, just need the text over-laid onto it..then convert to PDF.
The e-book will be an offer. I need five ads designed. I have the general text, I am looking for guidance on page design. The ads will be going out to an e-mail database we have. Respondents to the ad will go to a Landing page. I have samples of ads I DON'T like, and ones I DO like.
A Landing Page needs to be designed. It will have two boxes to fill in; Name and E-mail. The respondent will get the e-book if they supply that information.
