Wireframing and mock ups for home care nursing app

I am looking for a designer who can create wireframes and high-fidelity mockups for a home care nursing app. The app will be designed for both iOS and Android platforms. While I have a rough idea of the features and functionalities that I would like to include, I am open to suggestions from the designer. The wireframes and mockups should have a high level of detail, including detailed visual design and interactions. Ideal skills and experience for the job include:
